Consult with your physician regarding your participation in our smoking cessation program. E-mail Dr. Michael Pejsach letting him know that you have completed this step.

Ask your significant other, whether he/she is involved in the program or not, to E-mail Dr. Michael Pejsach a note stating that she/he will support your smoking cessation efforts. A short "I will support my spouse in her/his attempt to quit smoking," with signature, is fine.

Send an E-mail message with the following:
"I WANT TO QUIT SMOKING. I WILL THROW AWAY MY CIGARETTES BY OUR 3rd MEETING."

- After we reply to your E-mail (Step 3), with your acceptance or rejection, you will need to send a check with your payment of $100. Mail to Life&Health, 59 Monterrey Drive, Kenner, LA 70065-3142. Your fee covers up to six (6) on-line classes/meetings, all downloaded materials (Acrobat Reader will be needed), chat room, and up to three (3) private E-mail consultations. Three Components of our Basic Program will be used: a physical activity plan, "Care and Love of Self," and a "behavioral path" program.
After we receive your check we will send you the name and location of your support web site, health educator's E-mail address, and passwords to download Acrobat files. A schedule of "on-line meetings" with chat rooms and/or video feeds will be listed on the web page. "Classes/Meetings," will be PRIVATE meetings in which only participants will have access to the meetings.
- A Free Comprehensive Health Risk Appraisal (WellSource) computer scan sheet will be sent to you to complete after your check clears. You complete it. Mail it back and a confidential report will be returned to you within 3-4 weeks. All data is kept in a safe and not available to anyone.
