Here's your chance to participate in a 6-session e-mail/chat room educational Stress Lifestyle Adjustment Program (SLAP) . You will have an opportunity of learning how to enhance your lifestyle in three areas: relaxation (via on-line/downloadable relaxation tapes), communications skills, and time management. We will have special offers on related books, tapes, equipment, etc. A SLAP Workbook is available at no cost (a $15 value) to participants. The first session starts as soon as at least four (4) individuals, or two couples sign up. Additional sessions will follow.

Before we begin, you need the following Steps, in this order:

Send an E-mail message to Dr. Michael Pejsach stating why you want to participate in SLAP. This is the first step. By doing this, you acknowledge having read the "Selection and Claim Statements" below and that you understand, accept, and agree with all statements therein. There is no charge for this application process. |

Register by e-mailing the lead counselor: click here

Consult with your physician regarding your participation in the SLAP. Let us know that you've done this by e-mailing Dr. Michael Pejsach. |

Get a note from your significant other, whether he/she is involved in the program or not, stating that she/he will support your SLAP efforts. Please E-mail this as well to Dr. Michael Pejsach |

Complete the on-line application, with signature. You will get the web site address of this application after the above items are complete. Mail a hard copy of the completed application to SLAP, c/o Life&Health Enhancement Services, Inc. 59 Monterrey Dr., Kenner, LA 70065-3142. FEES WILL BE DUE AT THIS TIME. Please send your fee payment, via a check or money order, for the amount indicated on your application form, with your application, to Life&Health Enhancement Services, Inc. On line Visa and Master Card charging will be available soon. |
